Affordable Housing
- Shelter Plus Care – permanent supportive housing rental/utility assistance for the homeless and disabled. Case management services are provided and required for the program.
- HOME Tenant-Based Rental Assistance (TBRA) – housing rental/utility assistance for the homeless for up to 2 years. Case management services are provided and required for the program.
- ESG Rapid Re-Housing – short-/medium-term rental/utility assistance for the homeless (existing income source is required). Case management services are provided and required for the program.
- Permanent Supportive Housing – permanent supportive housing rental/utility assistance for the homeless and disabled. Case management services are provided and required for the program.
- SSI Reimbursement program - short-/medium-term rental/utility assistance for persons who have a pending SSI application to the Federal government.
- Financial Assistance – short-term emergency rent/utility/mortgage assistance for those facing imminent eviction with no other available resources.
Common Assessment Team (CAT)
HUD has mandated that all rental assistance program referrals for the homeless be coordinated through a centralized or coordinated intake system for the community. This coordinated system allows for homeless clients to be referred to the most appropriate housing program based on the client’s current needs and situation. The coordinated system also replaces the individual first-come/first-serve program waiting lists. In Louisville, our coordinated system is called the Common Assessment Team (CAT) and is administered by Family Health Centers (Phoenix Health). All homeless individuals and families should contact CAT at 502-637-2080 to arrange for a common assessment screening. Louisville’s CAT became effective on May 1, 2014. CAT referrals are required for Shelter Plus Care, HOME TBRA, ESG Rapid Re-Housing, and Permanent Supportive Housing program entry.
