Open Records Commercial Purpose Form

If your open records request is for commercial purposes, you must first fill out the commercial purpose form, have it notarized, and deliver or mail it to The Office of Management and Budget (OMB) before an open records request can be completed. The Metro Office of Management and Budget will contact you after your commercial purpose form has been received.

Download and print the Commercial Purpose Form (PDF File) and mail or deliver to:

OMB Finance Coordinator

Louisville Metro Government

611 W. Jefferson St., Louisville, KY 40202

You may also complete the general open records request online now and attach a scanned copy of the commercial purpose form as a file.  However, the request won't be completed until OMB approves your commercial purpose form. If you have questions, contact OMB at (502) 574-3211.
