If your open records request is for commercial purposes, you must first fill out the commercial purpose form, have it notarized, and deliver or mail it to The Office of Management and Budget (OMB) before an open records request can be completed. The Metro Office of Management and Budget will contact you after your commercial purpose form has been received.

With regard to this request, in accordance with KRS 61.870(4)(a)
“…the direct or indirect use of any part of a public record or records, in any form, for sale, resale, solicitation, rent, or lease of a service, or any use by which the user expects a profit either through commission, salary, or fee.”
Please have the officer of your corporation with signatory authority complete the “Certification of Commercial Purpose” form, if the records provided are to be utilized for a commercial purpose, as set forth in KRS 61.870(4)(a) as:
“…the direct or indirect use of any part of a public record or records, in any form, for sale, resale, solicitation, rent, or lease of a service, or any use by which the user expects a profit either through commission, salary, or fee.
