LouVax Scheduling

Currently, LouVax is vaccinating targeted groups and working with employers and healthcare partners to schedule individuals’ appointments. We will reach out to LouVax patients directly via email. Patients do not need to contact us. LouVax is not open for general public scheduling.




If so here’s what you need to know:

  • Currently our system only allows for scheduling appointments 10 days out. If you don’t see any available appointments within that 10-day window, it means none are available.
  • Rosters of targeted patients from tiers 1A, 1B, and 1C who are eligible to schedule an appointment at the LouVax site will be uploaded into our LouVax scheduling system.
  • These patients will receive an email from [email protected] and it will include “on behalf of LouVax Community <[email protected]>”
    • They will click the link in the email and will be routed to a secure login website where they will:
      • Verify identity via the email they/their workplace provided to Louisville Metro Government.
      • Create a password.
      • Fill out digital patient paperwork.
      • Schedule an appointment in the available time slots.
  • You can log back into your account at any time to manage your account and appointments. Follow the link in your welcome email or visit lmphw.force.com/login



How do I log back into my account?

Visit lmphw.force.com/login to log back in after you have followed all of the steps to create your account.

How do I create/reset my password?

Some users are experiencing issues with usernames and resetting passwords. Your username is either the email address that you signed up with, or if that doesn’t work, it is your email address plus “.louvax” at the end. For example: [email protected]

If you do not receive an email with password reset instructions after you have attempted to reset your password by entering your email address in the user field, please follow the instructions below:

Visit the LouVax login page at lmphw.force.com/login Click “forgot your password?”


If you continue to experience issues logging in, please contact the helpline at 502-912-8598 or email [email protected].

  • Enter your email address you used to register and add .louvax to the end of it (see example above)
  • Click the reset password button
  • An email will be sent to you with a link to reset your password
  • If you try logging back in but get an error message that you have been locked out of your account, please wait at least 30 minutes, then try logging in again with your username and new password.


Troubleshooting if you did not receive an email

If you believe you should have received an email to schedule an appointment, but did not receive it, first check your email spam folder. The sender will be [email protected] and it will include “on behalf of LouVax Community <[email protected]>”. You can also use the search tool in your email to search all mail for the words “Vaccine” “LouVax” or “[email protected]

Troubleshooting if you received an email

If you received an email from [email protected] (“on behalf of LouVax Community <[email protected]>”) and the system is not allowing you to schedule an appointment, please try the following actions:

  • Copy and paste the link from the welcome email or password reset email in the browser of a new tab instead of clicking directly on the link.
  • Use a different browser on your computer or if available, use your smartphone to visit the website instead.
  • If you are unable to click the link to log in, type https://lmphw.force.com/login Into your browser. This may allow you to enter your username and reset your password.
  • Type your email into the username field of lmphw.force.com/login and then click the forgot password link. Try with .louvax at the end of the username and without .louvax as sometimes it works one way and sometimes another way.
  • When entering in your Social Security number, include only the number with no dashes.
  • Don't use autofill to complete the demographics page. Type in the answers instead. If possible, turn off autofill in your browser settings.
  • Try refreshing the browser page.
  • When entering your location, use “Louisville” instead of your zip code, and search using a 25-mile radius to be sure that Broadbent populates correctly.
  • On the consent form page, be sure to click the "Yes" box for the "Do you consent?" question.
  • Be sure to e-sign and date the bottom of the Consent page.

As a reminder, LouVax will only be able to administer vaccines to patients with scheduled appointments.



How do I reschedule my appointment?

Please make every effort to make it to your scheduled appointment. If you cannot make your scheduled appointment and need to reschedule, please call the helpline at 502-912-8598 or email [email protected]

How do I cancel my appointment?

If you decide to not get vaccinated, please cancel your appointment by logging into lmphw.force.com/login and cancelling your appointment.

If you require further assistance or do not have access to the internet/computer, please call the helpline at 502-912-8598 to receive assistance.
