Please note that a drivers license, social security card nor passport are required to receive a vaccination. We will need your name and birthdate only. If you have insurance, please bring your insurance card but it is not required in order to be vaccinated.

Need help finding a place to get vaccinated? Need help making an appointment to get vaccinated? Need a ride to your vaccination appointment? Call the LOU Health COVID-19 Helpline at 502-574-8207. Someone is available to assist you Monday through Friday from 8 a.m. to 5 p.m.
 

Need a COVID test?

  • As of Sept. 25, 2023, you can order four free at-home tests sent directly to your home. Please visit covid.gov/tests to learn more information.
  • Beginning Monday, Oct. 2, 2023, Metro Parks will have free COVID-19 at-home tests in their community centers. Tests can be picked up during any community center’s operating hours while supplies last. Here is a list of community center locations. At home COVID-19 tests can be used on anyone 2 years old and older.
  • Get tested by your healthcare provider.
  • Find a COVID-19 test. Search by ZIP code and county​​​​​​
  • Check your local pharmacies for tests.  Some also provide treatment in addition to testing. Find a test to treat location​​​​​​.
  • Still need help finding a test? Call the LOU Health COVID-19 Helpline at 502-574-8207. Someone is available to assist you Monday through Friday, 8 a.m. – 5 p.m.
  • Parents can visit these Louisville program providers that offer vaccines for children.
  • Test2Treat.org is a program that provides free telemedicine visits for those who have already tested positive (home tests qualify) for COVID or flu. If they qualify, they can get an antiviral script sent directly to their home or local pharmacy free of charge. The program runs until the end of April 2024.
