24/7 Crisis Stabilization Unit: 502-561-1072

Provides mental health services and recovery for those with severe mental illnesses. Outreach provided through Assertive Community Treatment (ACT) team, a multi-disciplinary team with social workers, therapists, case managers, peers, and psychiatrists. The team supervisor is available to respond to the needs of homeless adults with symptoms of mental illness and/or mental illness and substance use disorders. The Crisis Stabilization Units provide 24/7 crisis stabilization for adults in psychiatric crisis. Services include therapy, case management, and psychiatric services (including medication).

Social or Specialized Services Outreach Groups 

Family Health Center- Phoenix

Contains the Louisville Metro Common Assessment Team, in addition to providing social service, case management, and mental health outreach. Medical care and housing assistance are also provided. The Common Assessment team is made up of multiple Metro agencies including Resilience and Community Services, Public Works, and LMPD Liaison Officers which work in conjunction with St. John Outreach. This group conducts an initial assessment when potential encampments are reported to Metro311 and also provides ongoing support, cleanings, and outreach for known encampments. 

 

St. John's Center for Homeless Men

St. John's outreach team connects unsheltered individuals to overnight shelter, recovery, social services, and healthcare.

The Forgotten Louisville

Serves a variety of encampments in different locations around Louisville every Wednesday night at 5 p.m. Provides a meal and supplies. Provides inclement weather runs during dangerously low or high temperatures or flooding. 

 

Keep Louisville Warm

Serves encampments in Okolona, Fern Creek, and Downtown Saturday afternoons. Provides supplies and a meal. 

 

River City Love Squad

Serves at 7 p.m. every Monday at 821 S. Clay St (corner of Clay and Breckenridge). Provides food, clothing, and hygiene products. 

 

Sidewalk Servants

Provides a meal and supplies on Friday evenings to various encampments.

Serves food at the Broadway underpass every Sunday at noon. Gives out clothing and supplies as well. There are mobile resource center routes that run Monday-Saturday.

Fern Creek United Ministries Street Reach
