Recovery and Mental Health Outreach Groups

Provides MAT and other addiction services, as well as mental health services. 24 hour bed access and peer support specialists available. 




Provides a variety of recovery options. 


The Healing Place 


Provides recovery options and 24 hour treatment. Peer support is also available. 




24/7 Crisis Stabilization Unit: 502-561-1072

Provides mental health services and recovery for those with severe mental illnesses. Outreach provided through Assertive Community Treatment (ACT) team, a multi-disciplinary team with social workers, therapists, case managers, peers, and psychiatrists. The team supervisor is available to respond to the needs of homeless adults with symptoms of mental illness and/or mental illness and substance use disorders. The Crisis Stabilization Units provide 24/7 crisis stabilization for adults in psychiatric crisis. Services include therapy, case management, and psychiatric services (including medication). 




MAT services along with a variety of recovery options. 


Kentucky Harm Reduction Coalition 


Provides psychiatric care, case management, recovery options and treatment, and harm reduction (Narcan and needle exchange). Works in conjunction Seven Counties and U of L.

Social or Specialized Services Outreach Groups 

Family Health Center- Phoenix 


Contains the Louisville Metro Common Assessment Team, in addition to providing social service, case management, and mental health outreach. Medical care and housing assistance are also provided. The Common Assessment team is made up of multiple Metro agencies including Resilience and Community Services, Public Works, and LMPD Liaison Officers which work in conjunction with St. John Outreach. This group conducts an initial assessment when potential encampments are reported to Metro311 and also provides ongoing support, cleanings, and outreach for known encampments. 


St. John's Center for Homeless Men 


St. John's outreach team connects unsheltered individuals to overnight shelter, recovery, social services, and healthcare. 


YMCA Safe Place Services 


24/7 Hotline: 502-635-5233

Outreach and crisis services for youth and adults aged 18-24.

Food and Supplies Outreach Groups 


The Forgotten Louisville 


Serves a variety of encampments in different locations around Louisville every Wednesday night at 5 p.m. Provides a meal and supplies. Provides inclement weather runs during dangerously low or high temperatures or flooding. 


Keep Louisville Warm 


Serves encampments in Okolona, Fern Creek, and Downtown Saturday afternoons. Provides supplies and a meal. 


River City Love Squad 


Serves at 7 p.m. every Monday at 821 S. Clay St (corner of Clay and Breckenridge). Provides food, clothing, and hygiene products. 


Sidewalk Servants


Provides a meal and supplies on Friday evenings to various encampments. 


Love Transformation Project 


Provides breakfast and clothing every Sunday morning in the Jude Catholic Church (4335 Hazelwood Ave.) parking lot. 


Burrito Riders of Louisville 


Serves breakfast burritos the 2nd and 4th Saturday of every month around 8:30 a.m. 


Fed with Faith 


Serves encampments and street homeless on Sunday evenings. Supplies and food is provided. Provides inclement weather runs during dangerously low or high temperatures or flooding. 


HipHop Cares


Serves food at the Broadway underpass every Sunday at noon. Gives out clothing and supplies as well. There are mobile resource center routes that run Monday-Saturday. 


Lost Sheep Ministries 


Serves food at 800 E. Liberty (Jeff St. Baptist) every Thursday at 6 p.m. 


Fern Creek United Ministries Street Reach 


Provides food and supplies to various encampments on Thursday evenings. Provides inclement weather runs during dangerously low or high temperatures or flooding. 


Feed Louisville 


Serve meals at various times during the week, as well as provide outreach services 7 days a week in the late afternoon and evening.
