Requesting Formula and Foods

WIC can provide noncontract formula and modified food packages when needed.* Before requesting a noncontract formula, check the list of WIC contract formulas below to see if WIC already offers a formula that is appropriate for your patient. If there is not a suitable choice, OR if you need to request any type of formula for a patient over 12 months of age (contract or noncontract), you’ll need to complete a request form. Monthly Formula and Food Benefits

WIC is a supplemental food program. This page lists how much formula and food participants receive each month. Most infants who are not breastfed will require more formula than WIC is able to provide each month. Maximum amounts of formula and food are noted below. To learn more about the WIC food packages, click here.

Infants (approximate amounts):

  • 0 – 3 months of age: 26 ounces formula/day
  • 4 – 5 months of age: 29 ounces formula/day
  • 6 – 11 months of age: 20 ounces formula/day, plus 24 ounces of dry infant cereal/month,
  • and 32 4-ounce containers of baby food fruits and vegetables/month

Children over 12 months of age and Women (amounts vary depending on participant category):

Approximately 29 ounces of formula/day if requested and approved, plus:

  • Eggs: 1 dozen/month
  • Cereal: 36 ounces/month
  • Cheese: 1 pound/month
  • Milk: up to 4 gallons/month (children approx. 13 – 17 ounces/day)
  • Juice: 1 gallon/month (children approx. 4 ounces/day)
  • Whole grain: 2 pounds/month
  • Beans: 1 pound/month
  • Peanut butter: 18 ounces/month
