Birth and Death Records

The Louisville Metro Department of Public Health and Wellness Office of Vital Statistics is located at 400 East Gray Street at the main administrative headquarters. Hours of operation are Monday through Friday, 8 am to 4:30 pm.


  • Declaration of paternity: The Department of Public Health and Wellness (LMPHW) can assist parents with obtaining a paternity affidavit by calling 502-574-6596 to schedule an appointment. Walk-in services are not available. Both parents need to be present and have an ID to complete the affidavit. Their signatures must be notarized.  LMPHW has a notary service we can provide at your appointment.  Another option to obtain a paternity affidavit is for parents to print the online Paternity Affidavit form, have it notarized themselves, then mail to the KY Office of Vital Statistics, 275 East Main St., 1 E-A, Frankfort, KY 4621.
  • Registration of births from home deliveries (By appointment only. Call 502-574-6596 to schedule.)
  • Provision of legal forms for funeral homes, nursing facilities, coroner’s office and hospitals
  • We do not have birth or death certificates of individuals at the Department of Public Health and Wellness. However, we do have applications to obtain them that can be picked up from our lobby.  After completing the application, you would mail it to the state of Kentucky’s Office of Vital Statistics (KYOVS) in Frankfort, Ky. for processing.  KYOVS also provides walk-in,  online and phone options to request birth, stillbirth, death, marriage, and divorce certificates. Requesting Birth or Death Information Prior to 1911?

If you’re seeking to examine records of births and deaths prior to 1911, they have been transferred to the Kentucky Department for Libraries and Archives, 300 Coffee Tree Road, Frankfort, KY, 40602.

Requesting Information on Genealogy Research?

Researchers may access the Kentucky Department for Libraries and Archives. The library is an excellent source of older documents and records when searching the family tree. The Kentucky Historical Society, PO Box H, Frankfort, KY 40602 has records that cover the period from 1851 to 1862, (502) 564-3016.
