Requirements to be a LMPHW Volunteer

Are you old enough to participate?

Volunteers must be at least 18 years old to volunteer in the clinics.

 

Have you submitted all the required forms?

Before you may begin volunteering, you must complete an online application. Once your application has been reviewed and processed, you will receive an invitation to complete a background check and a welcome email with additional forms that will need to be completed and returned to the Volunteer Center. These include a confidentially agreement and an Agreement to Volunteer and Accept Worker’s Compensation Benefits. All required forms must be submitted to the Volunteer Coordinator BEFORE you may schedule a volunteer shift.


Are you willing to obtain a TB skin test?

The Center for Disease Control (CDC) has recommended that healthcare personnel, including volunteers, be required to be tested for tuberculosis. You will need to contact the TB Clinic at 502-574-6617 to schedule an appointment to obtain your TB test. If you have been tested in the last year, a copy of the reading signed by a healthcare provider will satisfy the requirement for the skin test.

 

Can you commit to volunteering during your scheduled shift?

As a volunteer, we depend on you to complete your scheduled shifts. We do understand that from time to time certain situations may arise that prevent you from doing so. Failure to report or tardiness for a scheduled volunteer assignment without contacting the Volunteer Office can affect the ability of LMPHW to serve the public. Volunteers should make every effort to fulfill assignment commitments.

 

Is it possible to help without going to the clinic?

All clinic volunteer opportunities require that the individual work onsite. There may be virtual opportunities available through other departments. Please contact the Volunteer Office at 502-574-6179 for more information. 

 

Do you need community service hours?

Volunteers are asked for a minimum service commitment of 3 months with 8 or more hours of service per month. Community service hour verifications need to be submitted to the Volunteer Coordinator and are provided after 20 hours of service.
