Onsite Septic System Program
All work done to onsite sewage treatment & disposal system requires a permit from the Louisville Metro Department of Public Health and Wellness (LMPHW).
New Installation
To install a new septic system, the following steps are required:
- Submit a completed Application for Site Evaluation by email to [email protected] or by mail: LMPHW Attention: Onsite Program, 400 East Gray St 2nd floor, Louisville KY 40202
- Submit payment of $375 via check payable to Louisville Metro Government or credit card by calling 502-574-6655
- Once application and payment are submitted, contact septic installer to schedule site evaluation with LMPHW. Link to installer listing found below
- LMPHW and installer will meet to complete the site evaluation
- LMPHW will complete the site evaluation and email it to applicant and installer
- Installer designs system, submits drawings with grade shots to LMPHW
- LMPHW reviews submission from installer
- Applicant to submit $425 permit fee
- Once fee is received, an Onsite Sewage Disposal System Construction Permit is issues
- Once onsite sewage disposal system is installed all components must be inspected prior to being covered with soil
- All cover soil must be approved prior to use to cover the system
- Alarm if applicable must be in place and functional before the onsite system can be approved.
Building Plans Approved
Building/Construction plans cannot be stamped until an Onsite Sewage Disposal System Construction Permit has been issued.
- Submit construction plans via the online permitting portal
- Pay $25 plan review fee – invoice sent by email
- If the proposed onsite system is designed appropriately for the submitted building plans, plans will be approved
- If the proposed onsite system is not designed appropriately, plans will not be approved, and the onsite system will need to be redesigned

All requests for information on a septic system need to go through the Louisville Metro Government open records request portal. The following information should be included to better expedite your request:
- Address of Property
- Person Making Request
- Contact Information
- Subdivision and lot number
- Any Former Address (if applicable)
- Acreage of Property
