Please note: LMEMS cannot provide patient run reports via an open records request. Consistent with our obligation under federal health privacy laws to disclose private individual health information only when we have taken adequate measures to protect its privacy and security, we require that a patient or a patient’s representative must pick up patient care records in person and show a photo ID. A patient’s representative must also provide proof that he/she is entitled to obtain the medical record, through power of attorney or executor/executrix of estate documents, legal guardianship papers, or a birth certificate if the patient is under 18 years of age. Attorneys representing a patient may also provide a signed patient authorization or subpoena. To make a request for patient care reports, please contact the EMS Billing Department at 609 W. Jefferson St., or by phone at 574-4404.
