The Louisville Metro Department of Public Health and Wellness is coordinating the development of Healthy Louisville 2025--an action plan for improving community health. 

A Community Health Needs Assessment (CHNA) was conducted in 2018 among 3500 Jefferson County residents, with an additional 82 residents participating in focus groups. You can view the results online at https://louisvilleky.gov/document/2018communityhealthneedsassessmentdetailedfindigspdf 

Based on the input gathered from an initial kickoff event (which was held September 2018), along with the data gathered during the focus groups, community health needs assessment survey, the Forces of Change event, and the Health Equity Report, three major focus areas have been established: socioeconomic progress, environmental equity, and access to healthcare. A series of strategy meetings were held with partners across the community, where 4 strategies emerged:

  • Ensure equitable deployment of multimodal transportation options and review data to ensure improvements  contribute to health and quality of life.

  • Develop community-driven evidence-based interventions to promote green equity throughout Louisville.

  • Promote healthy housing throughout the life course for Jefferson County residents by developing a set of fair housing policy recommendations.

  • Align partnerships and community resources with the needs of Louisville families in the JCPS school system.

Healthy Louisville 2025 aims to identify opportunities to drive more attention, resources, or collaboration on ongoing projects already being done in the community or help to fill in gaps in areas that the community wants to tackle.

This project represents a public commitment to achieving a set of defined equity goals.  Louisville Metro Government expects these activities will foster a culture of health equity and build a movement toward ensuring everyone in the county can reach their fullest health potential.

LMPHW considers Healthy Louisville 2025 a living document and is committed to ensuring community voices are heard and drive the work.
