Louisville's Community Health Improvement Plan
A Community Health Needs Assessment (CHNA) was conducted in 2015 among 3500 Jefferson County residents, with an additional 82 residents participating in focus groups. You can view the results online at www.healthylouisville2025.com.
Based on the input gathered from an initial kickoff event (which was held September 2018), along with the data gathered during the focus groups, community health needs assessment survey, the Forces of Change event, and the Health Equity Report, three major focus areas have been established.
Healthy Louisville 2025 will focus on:
- Access to Healthcare (access to timely services, navigating the health care system, transportation, etc.)
- Socioeconomic Progress (Reliable employment, living wage jobs, paid sick leave)
- Environmental Equity (safe housing, built environment, empowering self-advocacy, environmental justice)
